We are currently recruiting Clinical Pharmacists on behalf of the Primary Care Networks we support. The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management across the network and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities. Harpenden Health Primary Care Network (PCN) is a group of 3 high achieving practices. We aim to provide a patient friendly quality service to our 45,000 patients. We enjoy multidisciplinary teamworking and the opportunities that collaborative working in a PCN provides.
We have a variety of roles available and successful candidates will be fully integrated into our clinical teams providing a holistic approach to acute care, chronic disease management, anticipatory case management and promotion of self-care across the three practices and within the community.
Supporting excellence in the care we provide in accordance with best practice, evidence-based medicine and targets will be essential as part of collective teamworking.
The roles (listed below) are expected to develop over time with engagement and supervision:
Experience in General Practice would be an advantage but is not essential for some of the roles mentioned above. Full-time and part-time options are available.
